Terms and Conditions

   

Eligibility

To qualify for transportation services provided by Let's Go Get'Em LLC through Uber, individuals must:

- Be an eligible participant under the New Jersey Division of Developmental Disabilities (DDD) program.

- Have an approved budget that permits the use of funds for transportation.

- Register with Acumen Fiscal Agent LLC and add Let’s Go Get'Em LLC as an approved vendor.

- Users must be at least 18 years old and legally capable of entering into a binding contract.

User Accounts

- You must register an account to access Let's Go Get'Em LLC services.

- You are responsible for maintaining the confidentiality of your login credentials.

- You agree to provide accurate, complete, and up-to-date information during the registration process and at all times.

Service Description

Let's Go Get'Em LLC coordinates Uber/ Lyft transportation services for DDD participants. The service is classified under:

- DDD Code: Transportation - Single Passenger

- Procedure Code: A0090HI

- Source: FI (Agency)

- Fiscal Intermediary: Acumen Fiscal Agent LLC

Payment & Budget Allocation

- 2.1. Clients acknowledge and agree that up to 60% of their approved DDD transportation budget may be used directly for Uber ride fares or mileage reimbursements, based on whichever portion of the budget is exhausted first. All transportation must be pre-approved by the Support Coordinator and reflected in the individual's NJISP. Usage is monitored to ensure compliance with DDD funding guidelines.

- 2.2. Let’s Go Get’Em LLC retains 40% of the DDD transportation budget as an administrative and service fee. This fee covers the following:

- Coordination and scheduling of rides through approved platforms (e.g., Uber and Lyft)

- Mileage, Budget tracking, and documentation

- Verification and reporting of trip details to the Support Coordinator or Case Manager

- Budget monitoring and alerts for usage thresholds

- Administrative overhead, including staffing, technology use, and records maintenance

- Communication with drivers, support teams, and families as needed

- Compliance assurance with DDD guidelines and Medicaid regulations

- 2.3. Let’s Go Get’Em ensures all transportation activities comply with the individual NJISP and that any service provided under this arrangement will not exceed the limits of the DDD-authorized budget. Clients can view their monthly reports summarizing transportation usage and remaining balances on the Uber app.

- 2.4. If a participant's transportation budget is fully expended before the end of the service period, no additional rides will be scheduled unless authorized by the Support Coordinator or funded through alternative arrangements. Any unused portion of the 60% budget will be forfeited and applied toward service fees. These funds are non-transferable and non-refundable.

Terms and Conditions: Tipping Policy for Uber or Lyft Services

- Please be advised that gratuities (tips) for Uber drivers are not included in the DDD-approved budget and therefore cannot be covered or reimbursed by Let's Go Get'Em LLC or the Division of Developmental Disabilities (DDD).

- All tips provided through the Uber app or in person are considered personal expenses and will be charged directly to the personal debit or credit card associated with your Uber account.

- If you choose to tip your Uber driver, please understand that the amount will be charged to your personal card and will not be eligible for reimbursement through the DDD budget or any funding administered by Let's Go Get'Em LLC.

- By participating in Uber transportation arranged through our services, you acknowledge and accept these terms regarding tipping.

Mileage and Budget Monitoring Policy

- The individual's monthly mileage and budget allocation are actively monitored to ensure that resources are utilized appropriately and consistently across the entire duration of the service plan.

- Each individual receives a set amount of miles and/or funds (“mileage/budget”) designated specifically for transportation services monthly. If the individual exceeds their monthly mileage/budget allocation, they will not be permitted to receive additional rides beyond the allocated amount for that month.

- In such cases, services related to transportation will be temporarily placed on hold until the next monthly cycle, when a new mileage/budget allocation is issued. This process is intended to help individuals maintain access to transportation services throughout the full span of their support plan without prematurely exhausting their resources.

Vendor Authorization & Invoicing

Participants must add Let's Go Get'Em LLC as a vendor via Acumen Fiscal Agent. Participants or their Authorized Representative must approve all invoices submitted in Acumen for services rendered. Let's Go Get'Em LLC, in partnership with Acumen, provides Uber transportation services as an approved support option under your service plan. All invoices related to transportation must be signed and approved in accordance with Acumen’s billing requirements, regardless of actual usage.

Invoice Notification: 

- Let's Go Get'Em LLC will notify participants or their Authorized Representative when an invoice has been submitted. Notifications may be sent via phone, text, or email.

Invoice Approval Requirement

- Invoices for transportation services are generated and submitted on specific billing deadlines established by Acumen.

- Even if the individual does not utilize their Uber transportation during a billing cycle, the invoice must still be signed and approved.

- This process ensures compliance with Acumen’s payment system and prevents delays, backdating, or disruption in services.

Service Availability

- Once transportation is approved and invoiced, the Uber service remains available for the individual to use during the approved service period.

- Failure to use the service does not result in a credit, refund, or adjustment to the invoice.

Client/Guardian Responsibility

- Clients/guardians are responsible for reviewing each invoice provided and returning signed approvals by the designated due date.

- Non-compliance with invoice approval deadlines may result in suspension of transportation availability until the matter is resolved with Acumen.

Acumen Billing Compliance

- Acumen requires strict adherence to invoice processing timelines.

- Let's Go Get'Em LLC does not have the ability to alter or delay invoice deadlines once they are issued by Acumen.

- Approval of invoices is a condition of continued access to transportation services.

Non-Usage of Services

- Choosing not to use Uber transportation during an invoice period does not eliminate the responsibility to approve invoices. Failure to approve invoices may result in service interruption or cancellation.

Ride Booking and Use

- Ride requests are subject to driver availability.

- Only the authorized DDD participant is eligible to use the ride.

- You may cancel a ride under specific conditions outlined in the cancellation policy.

- You are responsible for being ready at the pickup point at the scheduled time.

- Misuse of services may lead to suspension or termination.

Cancellations and No-Shows

- Cancellations must be made at least 1 hour before the scheduled ride.

- Fees may apply for cancellations made after a driver has been assigned.

- No-show riders may be charged a fee, and the cost will be deducted from their DDD budget.

- Repeated violations may result in termination.

Prohibited Conduct

- Unauthorized riders

- Threatening or abusive behavior

- Misuse of DDD/Acumen funds

- False or misleading information

Service Disruption

Let's Go Get'Em LLC reserves the right to suspend or cancel service due to non-payment, policy violations, or eligibility issues.

Text Message Agreement

By enrolling in Let’s Go Get'Em LLC services, you consent to receive texts related to:

- Invoices and payment reminders

- Service updates

- Ride scheduling

Standard messaging rates may apply. Opt out via text message.

Terms and Modifications

These terms are valid for one year from the date of agreement. They are subject to change at any time. Updates will be shared via email, text, or posted on the website. Continued use constitutes acceptance.

Agreement

By using our services, the participant and/or Authorized Representative agrees to all terms, including payment responsibilities, riding conditions, and communications.

Limitation of Liability

Let's Go Get'Em LLC is not liable for indirect, incidental, or special damages. Uber assumes responsibility for transportation incidents. Total liability is limited to amounts paid to Let’s Go Get'Em LLC in the last 12 months.
